How to Register

If you would like to register with Priory Gardens Surgery, please click the register now button below. You will only need to complete the * sections of the registration form.

New Patient Process

Step 1: Complete the * sections of the registration form on the next page (click register now to access this page). 

Step 2: Your details will be verified by the practice, if further information is required, you will be contacted via SMS text message which you will be able to respond to with the required information. 

Step 3: You will be sent your login details for online access and any relevant forms you are required to complete. You will need to activate this within 24 hours of receiving the link.

Step 4: Once you have logged into your online services, you will need complete the sections as specified in the SMS message you will receive as part of step 3.

Your registration will be complete once all steps above have been completed. You will receive a confirmation welcome SMS within 24 working hours of completion.

Please allow 5-10 working days following the completion of the steps above for your registration to be activated.

 

Out of Area Scheme

New patients who live outside the surgery area and existing patients who move away can request that they remain registered with the surgery as an Out of Area Patient.

Patients who are in the scheme will NOT be eligible for home visits.

All applications will be considered and we will endeavour to register any patient irrespective of their address. However there may be times when an existing condition and the lack of home visits may pose too great a risk to the ongoing care and we would have to decline these applications.

 

Temporary Registration

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient visit the practice. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
